Web Form registration and approver screen send mail in .net

Question

I needs to create a web registration form like Name,Age,address,country. once the user submits details a mail needs to trigger to approver with approval screen link with values entered by the user. once the approver approves it then needs to change the status in database as active. can anyone help to create a page in this scenario.
