how to create Menu options after click on that

Hi every one,

I am facing this issue i want to create a page in which there are menu and when we select any option from that menu.

i want this type of the screen. I am attaching a immage please check it and give me a solution of that.

and in one page there are the option for adding that details and show those thing in respective's menu.

please any body help me.
