insert into mysql [XAMPP] from .dbf

Help me solve this problem

I have input the database .dbf and i have display it in datagridview like this:

 

 

 

 

What I ask is how from .dbf to sql. I have tried to create a similar table but tetep can not be input. How from datagridview in save to sql.

I do not understand to solve this problem
