How to read csv file vertically?

Question

I tried lots in lots of ways but not reached to answer.

my question is i want to read csv file in the following format ....

 

ID=,10001,

Name=,Hemant Desai,

Age=,60,

Sex=,male,

Doctor=,Pathak,

Mobile=,9021412202,

Alignment=,brain toumer,

No of medicins=,3,

12:02,Stamlo-5,1mg,oral,after meal,*XE0280916*

12:01,Losar,3mg,oral,after meal,*XE0284350*

Respiratory Rate =, 999

in this file the data is in all headers and its values are in vertical format which keep this hard to read and the data from

No of medicins=,3,

should be in one column No of Medicins . can not change format or remove quammas as this format is fixed.

and save this data in table "Medicin" as in respective columns as (((id , name , age , sex , doctor , allignment , no of medicin , temperature , blood pressure , pulse rate , respiratory rate ))).
