Visual Studio 2013 Login form SQL with phpmyadmin

Im sitting here for 5 hours and try to Connect my PHPmyAdmin with my Tool in #C but i cant.

 

All Videos on youtube are false and not working.

 

Have anyone here a Code for Login Form?

 

also that i can Connect my  phpmyadmin table to the form.
