How can i create a workflow programatically without using SharePoint designer and toolbox in Visual studio

Is there a way to create a workflow from scratch with all activities?

No, It is not possible, You need to take help of SharePoint Workflow Engine to create your own Activity or you have to depend upon Windows workflow foundation,
