Crystal reports

In a case the patient id is provided by the end user with the help of textbox from UI. based on the id the patient record filtered from multiple datasources and displayed on the crystal report with their datasource Name as Caption, one by one on a single page, with respect to the patient id.

as given below

DrReffered: AGE: Gender:

Add a query/dataset with parameterized on patientID.

Plant labels/textboxes as per reporting requirements. Thats it!!!
