Divert Network Traffic from specific port to specific IP/PORT

Hey all,

I'm loooking for some information on taking incoming traffic on a specific port sending it some to another IP address, and then back again.

So would be similar to a proxy server but thats not the goal.

Client -> My Sever -> Destination -> My Server -> Client

Would appreciate any help.
