get selected value of SP 2013 radio button on change event

Use that id in the code.

It is a custom list New Item form which is a customized one. Created this custom New  item form using sharepoint designer 2013.

 

It is not a modal and it has  a sharepoint radio button field for which i required on change event to get the selected value.

Can you please tell me that your custom request form is a modal or something else? so, that I can understand how it looks like.
