Select SharePoint Form from Tree View from left side.

select Customize with Powerapps option from upper right corner. Your form will open in Powerapps then you can have to publish it so new powerapp form will show instead of OOTB sharePoint list form

JSON will work on only in Panel view of New form but to achive it on New Item page url you need to customize the form using PowerApps.

Hi,
You can change the layout of the pageusing JSON:

Select Header in Dropdwon.

Paste below JSON with adding your required text content in txtContent property:
{
    "debugMode": true,
    "elmType": "div",
    "txtContent": "Your Text Here",
    "style": {
        "height": "24px",
        "font-size": "18px",
        "font-weight": "600",
        "padding-bottom": "10px"
    }
}
Your Header Text will be changed.
