How to add Survey List Rating text description for 7 Ratings

I have created a Survey list.Now i want to give description to each of the ratings used in that survey list.How to do that? i already tried this with adding a script in sp designer but it doesnt work.Can anyone help me on this?

Could you please explain in which form you want to give description to each of the rating used in that survey list ?

Example : For NewForm.aspx; DispForm.aspx; EditForm.aspx ?

That's right you are using Script but make sure your code within the content tag with contentplaceholderid="PlaceHolderMain".

Still you are facing same problem , then please let me know. i will explain you step by step .
