How to create a Wizard?

Hi,

I want to create web wizard. The objectives are:

1. it has 20 questions to be set in 5/10 pages.

2. The answers will be either a drop-box, Checkboxlist or a Textbox.

3.A user selects his answer and moves to the next page.

5. All the answers should be stored in a table in DB.

6. User gets a follow-up e-mail thanking him to answer the questions.

I want to know what all I need, presentation layer, DB and??

Where do I start? Any projects OR examples??
