How to create setup file in vs 2012 with sql server 2008R2

I am using Visual Studio 2012 and SQL SERVER 2008 R2. I want to create setup file (Windows Application) for Client Machine. I have successfully created Setup in VS2012 using InstallShield without database and its working fine on client machine. Now I want to create setup with Database so that my application can insert,delete,update records from database and also need to create Crystal Reports. Plz anybody explain me steps for creating setup with database including config setting and all.
