How To make a Complete SetUp exe in Visual Studio 2010 C#

Hi I am Making a setup, and i make this, but when i am going to install this setup
it is not created in my desktop,and start button,it just create in Program file folder in disk c.

means i want to make complete setup that when i install this setup then it create in my desktop and in start button also.

and how i can add icon on this,

means i want to add an icon on this setup whose look like very good,

please help me for this,
i am new in C# windows.
please i request you don't go on my english,and help me please
