That depends on what Data Access mechanism you are using. If you are using the traditional ADO.NET, you could do something like this:
private string GetConnectionString(){
  return System.Configuration.ConfigurationManager.ConnectionStrings["DBConnectionString"].ConnectionString;
}

private int InsertRecord(string name, string age, string gender, string city){
        int affected = 0;
    string sqlStatement = "INSERT INTO YourTableName values(@name,@gender,@age,@city)";

            using(SqlConnection connection = new SqlConnection(GetConnectionString())){
                using(SqlCommand cmd = new SqlCommand(sqlStatement ,connection)){
                        cmd.CommandType = CommandType.Text;
                                cmd.Parameters.AddWithValue("@name",name);
                                cmd.Parameters.AddWithValue("@gender",gender);
                                cmd.Parameters.AddWithValue("@age",age);
                                cmd.Parameters.AddWithValue("@city",city);
                        affected = cmd.ExecuteNonQuery();
                }
        }
    return  affected;

}

protected void Button1_Click(object sender, EventArgs e){
       //perform forms validation check here
InsertRecord(txtname.Text,txtage.Text,ddlGender.SelectedValue,ddlCity.SelectedValue);

       if(result > 0)
        Response.Write("Data inserted successfully!");
