How to Signout And Redirect to A custom Page SHarepoint

I have developed a Change Password Application Page(Custom page).I want after Click On submit button it should Singout and Redirect to Signout Page(Custom Application Page).

How to do this ?

I am using the below Configuration. Sharepoint 2010 Visual Studio 2010(3.5)

I have already configured FBA Authetication in My application.

Are you using Forms Authentication or Windows Authentication?
