How to upload videos in php and store name in db?

I want to post videos as a user and as a admin i want to add and manage videos.That videos i want to store name in a database.Please let me know as soon as possible.

upload video file into the folder and save name of video and path in database. And bind the path from database and on click open the path where video is save.
