modal popup on button click with file upload and get path

hi

i have a button in my view i want to open modal popup on button click with file upload controll on it and a upload button. i want to call action method o upload button click and get file url,

after resding the file i want to return some data on the same popup to display how many row inserted in tabl. and use ajax call.
