Building a PACS server with a viewer can be a very difficult undertaking and your best bet is to start with an SDK that implements the DICOM standard and build your system. DCMTK is a free DICOM toolkit which you can use to build your server:

The other option is to consider using a commercial tookit like the LEADTOOLS SDK.
