Placing Toolbar at the form

yes now toolbar  & status bar is visible in the Toolbox .but in the following code .when i run it says
index was out of range .Must be Non-Negative and less than the size of the collection .Kindly let me know .
any idea. any help would be highly appreciated.

Right click on Toolbox-Choose items-In .net framework components -Add toolbar(check all the toolbar components checkboxes on left)
