Sometime record inserting two time in sql table.

Question

the same record inserting two time in the sql table Sometimes . But I am inserting only one record. I have successfully build the software and this project is live now and working fine. But this issue happen sometime with online users. I have checked code many time but i diden't get this issue. I have develope this project in asp.net with sql databse. Kindly help.
