VBA basic scrip in html to send form data to excel sheet

Hi All,

 

I hope i have posted this in the right section?

 

What i want this htm file to do is allow me to fill in the serial number highlight 1 of the radio buttons then a selection of any of the other check boxes.

 

Once i hit add record at the bottom that then should open up the excel sheet (currently sitting at "C:\Users\ssims1\Desktop\TEST.xlsx"), put the details from the form in the next new line in excel and then save.

 

I have see it working on a smaller scale and thats were i got the basics of the code but just cant get it to work with the radio buttons or check boxes. (serial number works fine)

 

Any advice greatly received
