Blend is mainly used to develop user interfaces ,it includes set of tools to make UI development easier.Visual Studio 2012 and blend is all you need to develop windows 8 applications.
To develop windows phone apps you need to intstall Windows Phone 8 SDK.Once intstalled you can develop windows phone apps from within visual studio.Please let me know if this answers your question.
