I am new to share point kindly some one guide to how to create below 

task , 

Objective	To create a forum/Platform where all associates of Global Data Compliance could make use of it to openly post/share their feedback for Overall Process improvement ideas.
Purpose	Fields Required in the Form	Field Type
Associate Details	Request Number	Auto Populate (Unique)
	Associate Name	User Field
	Email	Text Box
	Date	Date and Time
	Process	Drop Down
	Technology	Drop Down
To attach any supporting document	Attachment	Attachment
Stakeholder	Stakeholder	User Field
For Stakeholder's response	Stakeholder or Managers Comments/Feedback	Text Box
Any supporting Attachment from Stakeholder	Attachment	Attachment

 

person once create new request they will not be able to edit the request and it should be visible only to associate and Stakeholder. They should see only their own request (Requests created by other shouldn't be visible).  please guide me some one
