This examples shows how to merge two separate files. Its load first file and second file. But if you replace second to first, than it would be exactly what you want.

You can do that using specialized toolkit. Using LEADTOOLS you can copy the original file then combine the new copy to the original file. The code is very simple like this:
Leadtools.Pdf.PDFFile firstPdf = new Leadtools.Pdf.PDFFile("Original.pdf");
firstPdf.MergeWith(new string[] { "Copy.pdf" }, "Final.pdf");
You can repeat these lines several times to duplicate more pages like this:
for (int i = 0; i < 4; i++)
{
PDFFile original = new Leadtools.Pdf.PDFFile(@"Original.pdf");
original.MergeWith(new string[] { @"Copy.pdf" }, @"Copy.pdf");
}
