You can replace an image inside an existing word document through use of the ImageEngine. The ImageEngine is a subcomponent of the main DocumentEditor which will allow you to edit the images. Here is some sample code and documentation which demonstrates this:
addImage = () => {
    //Allows the user to input an image of their choosing for the specific file types allowed at the cursor location
    var input = document.createElement('input');
    input.type = 'file';
    input.style.display = 'none';
    input.accept = '.png, .jpeg, .tif, .docx, .pdf';
    input.onchange = function (e) {
        var files = input.files;
        if (!files || !files.length)
            return;
        file = files[0];
        var url = URL.createObjectURL(file);
        editor.engine.images.insertImage(url);
    };
    input.click();
}

You can also use spire.doc to replace image in word document, it would be easier than using open xml.
