PowerApps Radio Button Yes/No with CommentBox

Hi All,

 

 

could you please help me PowerApps below Scenario.

 

I have Radio button with yes/no

 

if I select No it will give you a comment box I want to submit this comment data in a sharepoint list. 

Here i am using patch function in powerapps
