Login Form (Username +Password) connecting with Mysql database in C#???

Question

Please provide me with information or code on how to
connect a login form (with username & password) with
mysql database??
The really problem is to create a login form in my pc and connecting this with
a mysql database in my pc too...
1)How do I achive that??
2)What tools I need except visual studio and mysql software??
3)What's the appropriate code to conncet my form with database above???
