Complete code for a Form with 2 text boxes

Good day,

 

can anyone provide me please with the code for a windows form that has below format

A windows form with a button that when pressed spawns a new form as per below: 

 

 

textbox 1 and 2 need to be read in and assigned to variables

Button needs to execute a windows command which implement the 2 variables
