how to create popup window??

In my application I am adding login button.

When user click on it popup should come with ok button for uname and password.

On popup after filling uname and password after clicking on ok it redirects to previous page with uname and password.

How to create popup? and how to redirect to previous page?

1. Create a form, place the user name and password text fields in it
2. From the Parent, create a object for the above form
3. Then call ShowDialog() member function from the previously created object.
