how to create the template like this

create a page.. give it layout like below using styles..

now according to your no. of images in your textboxes. user enters add the those no. of file upload and image control to your asp panels programmatically..
for eg:

if user enter 1 for top panel then:

for(int i=0;i{

pnlTop.AddControl( new Image());
pnlTop.AddControl(new FileUpload());

}

similary you can access them by using foreach loop:

foreach(Control c in pnlTop.Controls)
{
   if(C is FileUpload)
    {
        // write your custom code here..
    }

}

I think now you'll have some idea to achieve your task.
