How to insert value of 2 dynamic dropdownlist and 1 textbox

How to insert value of 2 dynamic dropdownlist and 1 dynamic textbox value in sqlserver database.

I have 2 Button.

one is for add row,second is for save

when i will click on add button then 2 dynamic dropdownlist and 1 dynamic textbox added.

but dont have code how can i insert that record into sql server database.
