How to make questionaire form (Approx 20 question) ?

Dear Champs,

I want to make a questionaire form (Approx 20 questions) in two forms and store in single database table?

What is the procedure ?

Please guide me :-(

first of all decide which form your going to use
1. webform or windows form.
then,
in both cases, as per your requirement you need following windows,
login
questionaire
report
to save questionaire in database use following fields in single table,
Qno as integer Primary key
Question Varchar() or text
AnswerVarchar() or text
UserID int
RightAnswer Varchar() or text
In design,
Either you can use panel or keep controls on forms,
