How to Make Questionaire in MS Access 2007 ?

Dear Champs,

 

How to make a questionaire (Survey/ Case History) large Size Field  in MS Access 2007  and implement the database in c# windows form.

 

For E.g. of Question :

 

This is the Survey question :  

 

Q. Exact details of every complaint as to how it started. a probable cause leading to it e.g. emotional trauma any other acute disease any other medicine intake  ?

 

this question is more the 255 length size how can i store this text field in ms access 2007.

 

Please guide me :-(
