SharePoint 2016- Customize form

Hi All,

Need help on below things:

There is an image when i click on that , it will be redirect to open a form (registration) and once submit it will be saying "Thank you message " and close the form.
And all items are stored in custom list.
once item submitted logged in user will get email.
Any reference please share ASAP.

You can use the Microsoft Forms webpart if you have Office 365 license.

Another option would be to use the SharePoint Designer 2013.

Using SPFx is always an option if you are open to write some code.

Thanks for the response, but i am using SharePoint 2016 On-prem version. Still we can use?

Have you explored an option of using Microsoft Forms? I think it will be a good fit for this requirement.
