Simple User Register Form in asp.net Mvc or Entity Framework

Question

I am creating One user Register Form But I have 4 Doughts?

 

1)In this I am trying to Implement a dropdown also i.e  cascading drop downs and we are take separate table this Casding drop downs or not?

 

ex:Country, state, City 

 

2)Radio Buttons i.e Male and Female select the radio  the data is not inserted 

 

3)Check box i.e  select the check box the data is not inserted

 

4) This is my internal dought is what are the data types  we are take by salary and date in the sql and c#
